4 DAY EXTREME SMILE MAKEOVER

The following requirements are for those interested in this amazing process.

Pre-requisites for Consultation:

1.   Download and completely fill out the Smile Evaluation Form. Fax to 305-279-2649 when completed. (scroll down to download forms)
2.   Send by Email 3 digital pictures of your smile. The doctors will evaluate the photos, review the Smile Evaluation form and submit the report within days for your next step.
The following 3 pictures show examples of acceptable angles and clarity. Try to achieve pictures close to the ones being shown.

Once the preliminary information has been presented based on the photos provided, then you the (patient) will be more informed to make a decision of when to plan your Extreme Smile makeover visit.

An approximate fee based on the photos, telephone discussions and other information shall be presented. This fee will obviously fluctuate once the full and comprehensive examination work up is created on the first day of your visit.

There will be secured deposit of US $2,500 for this first day and its related information previously evaluated.  This entire fee will be applied to the case fee once the patient agrees to go forward with this case. If by the remote chance, the patient does not want to proceed, US 1,000 will be refunded by the same means in which it was provided within 48 hours.  The remaining US 1,500 will be the fee for all the preliminary work and evaluation prior to the visit and including the examination process.
